The School of Social Work at the University of Illinois at Urbana-Champaign is pleased to assist you in obtaining an Illinois Type 73 School Service Personnel-School Social Work certificate. In order to do this, you must submit the following:
- Post-MSW Application. Detailed descriptions of learning experiences completed as part of your school social work internship are required.
- UIUC Entitlement Request Form. Use a non-university email address that you check regularly.
- Original or notarized copies of score reports from National Evaluation Systems indicating that you passed the Basic Skills and School Social Work exams
- Electronic version of your portfolio. For more information about portfolio requirements, contact the appropriate faculty member at your institution.
- Dr. Jan Wilson, St. Louis University
- Dr. Carol Doehlling or Dr. Melissa Jonson-Reid, Washington University
- Pam Long, ACSW, LCSW, St. Ambrose University
- Official graduate transcripts that verify the date you received your M.S.W. degree. If you completed coursework required for school social work certification at a different institution, you must send that transcript as well. Transcripts issued to students will only be accepted if they are received in a sealed envelope with university insignia.
- $30 check made payable to UIUC School of Social Work. This fee will cover the cost of reviewing your records for your certification. There is an additional charge for your Type 73 certification payable to the Illinois State Board of Education.
- School of Social Work Post MSW Certification Application Checklist with your signature verifying that all materials have been sent.
All materials (except for official transcripts) must be submitted to the School of Social Work at the same time. The deadline for May 2008 graduates is May 15th. Failure to submit the necessary documents by the requested deadline may result in a delay in receiving your Type 73 certification. In addition to submitting the required documentation, you must create an Educator Certification System account at https://secqa1.isbe.net/ecs/ . After your application for certification is approved, you will receive an email with instructions to go logon to your ECS account and pay for your certificate electronically with a credit or debit card.
